16 hours per week – fixed term until December 2025
The role
Auckland PHO is looking for an outstanding person to join our team who has great community connections and understands self-management principles, especially for people with diabetes.
Auckland PHO holds a contract to deliver Diabetes Self-Management Education (DSME) sessions across Auckland for general practices and community groups.
Working alongside our Self-Management Coordinator, you will be responsible for:

Coordination the delivery programme and where required, facilitate self-management sessions

Preparing education sessions by arranging venues, resources and communications

Identifying and reaching out to community groups, Iwi and Māori health providers, Pacific health providers and other health providers who deliver diabetes health services

Monitoring the quality and success of the programme

Developing and facilitating DSME facilitator training as required

Promoting DSME sessions

Responding to enquiries, requests and promotional opportunities
We are looking for Kaimahi who:
-
Has a background and experience in working with communities with experience in self-management education
-
Understands type 2 diabetes
-
Knowledge of adult education
-
Experience in self-management and behaviour change skills
-
An understanding of the Primary Care sector
-
Has a positive attitude, is a team player, able to work independently and as part of a team.
