Kia ora Practice Teams

COVID-19

As you are all aware, there are changes to the Model of Care Funding from 1 October 2023.

There is also a presentation video from the team describing the upcoming changes available here. password is: careinthecommunity (video is 22minutes)

We recommend you continue to review your Practice COVID-19 Positive Daily Risk Report as the source of truth for any patient testing positive for COVID-19 to assist with their management, acknowledging that there must be clinical discretion applied. Individual patient reports in a GP’s inbox do not necessarily provide all the information in one place to enable a clinical decision to be made.

The information is derived from patient demographics, co-morbidities, hospital admission data and dispensed medications and the Mōhio team provide the report for your practice.

If you are unable to download or view your practice report, please contact tracey@aucklandpho.co.nz or ashley@aucklandpho.co.nz

Immunisation Reports

An email was sent to Practices on Friday outlining new immunisation reports – childhood immunisation and Bexero Catch up for under 5s to assist with recalls. If you missed it, see attached.

Changes to Mōhio forms and reports can be located on the Auckland PHO website – resource library | password: aucklandpho

Contact Shanaz@aucklandpho.co.nz if you need any further support with these reports.

Welcome to Siobhan Matich to the Auckland PHO Team

As you know, Alison Brown is leaving Auckland PHO soon to pursue travel adventures. We are delighted that Siobhan has joined the team as our Clinical and Quality Improvement Manager. Siobhan has had a long career in health and has worked in front line general practice, nurse leadership and in private corporate roles and therefore brings a wealth of experience to support our general practice teams as well as the Auckland PHO team. While we are sad to farewell Ali, it’s great to welcome Siobhan to fill the gap that Ali leaves and you will get to meet Siobhan soon.

Ngā mihi,

Barbara Stevens |Tumu Whakarae/CEO
