Care Plus

Introduction

Care Plus is a Ministry of Health funding programme for PHOs and Primary Care, designed to provide low cost access for people with high health needs. The criteria for eligibility for this programme is primarily focused on those patients that are expected to need at least two hours of clinical time over the ensuing six months. The goal is to improve health outcomes for those with chronic illness and to promote improved and more effective utilisation of available health care services.

Patients with a High User Healthcard (HUHC) CANNOT be enrolled onto CarePlus as the practice already receives an increased funding for these patients.
